Substance use disorder (SUD) is a complex condition in which there is uncontrolled use of a substance despite harmful consequences. People with SUD have an intense focus on using a certain substance(s) such as alcohol, tobacco, or illicit drugs, to the point where the person's ability to function in day-to-day life becomes impaired. People keep using the substance even when they know it is causing or will cause problems. The most severe SUDs are sometimes called addictions.

Re: Buprenorphine for Pregnant Individuals with a Substance Use Disorder Diagnosis

The Department of Healthcare and Family Services (HFS) is providing this update regarding buprenorphine dosing for pregnant individuals with a substance use disorder (SUD) diagnosis, who, due to growing fetus and maternal weight gain, are likely to require a higher dosage than the usual 24 mg maximum dose.

HFS is working on programming to allow a total daily dose of up to 32 mg of all buprenorphine products (monotherapy as well as combination products with naloxone) for pregnant individuals with SUD without a prior authorization, when both diagnoses (pregnancy and SUD) are noted on the prescription.

These services will be covered under both the fee-for-service (FFS) program and managed care organizations (MCOs) with a target implementation date of February 1, 2024.

Questions regarding this notice may be directed to a pharmacy consultant in the Bureau of Professional and Ancillary Services at 877-782-5565 for FFS claims, or the appropriate MCO plan.

What are the types of heart disease that women should be aware of?
1. Coronary artery disease is the most common kind of heart disease.
2. Heart rhythm abnormalities, such as when the heart beats irregularly (i.e., atrial fibrillation), which can lead to blood clots and complications like stroke and heart failure.
3. Heart failure: See the definition. Causes of heart failure include heart attack, chronic conditions like high blood pressure, excessive alcohol use, and some viral and bacterial infections.
4. Congenital heart diseases. They exist at birth. They could develop over time due to infection, disease, or other factors.

What do the statistics reveal?
In 2024, 60 million American women were living with some form of heart disease.

Coronary artery disease

Coronary artery disease: What is it?
The arteries in the heart narrow or become blocked by plaques made of cholesterol deposits. Coronary artery disease and vascular disease, or disease in blood vessels, are the leading cause of heart attacks and strokes.

What are the risk factors for coronary artery disease?
1. High blood pressure
2. High cholesterol
3. Diabetes
4. Obesity

Cholesterol: What is normal?
Normal range of cholesterol: Less than 199 mg/dL

Obesity: How is it diagnosed?
Obesity/body mass index (BMI) over 30
In general, the following BMI ranges (in kg/m2) classify different weight types:
Underweight: Less than 18.5
Optimum range: 18.5 to 24.9
Overweight: 25 to 29.9
Class I obesity: 30 to 34.9
Class II obesity: 35 to 39.9
Class III obesity: More than 40

How do you calculate your body mass index?
You can calculate your BMI yourself by following these steps:
Multiply your weight in pounds by 703.
Divide that answer by your height in inches (there are 12 inches in 1 foot).
Divide that answer by your height in inches again.
For example, a person who weighs 180 pounds and is 5 feet 5 inches tall (65 inches total) would calculate their BMI in the following way: 180 x 703 = 126,540.
126,540 / 65 = 1,946.769.
1,946.769 / 65 = 29.95.
The person’s BMI is 29.9.

How does heart disease affect women differently than men?
1. The size and structure of the heart is different in men and women.
The process starts at birth.
Women generally have smaller hearts and blood vessels than men. Women have a greater likelihood of developing heart disease in the smaller arteries of their hearts. This is harder to diagnose compared to problems with the larger arteries and contributes in part to the higher rates of missed diagnoses in women.

2. The estrogen levels that occur in women during their lifetime may affect their risk of coronary artery disease.

3. Women are more likely than men to have certain conditions that increase their risk of heart conditions, including anemia and endometriosis.

Pregnancy and cardiac problems: What are the issues?
1. High blood pressure
2. Gestational diabetes
Women of childbearing age need to be aware of these conditions and pay attention to heart health before, during, and after delivery.
3. Enlarged heart resulting in heart failure

What symptoms should women be aware of for cardiac emergency or heart-related medical emergencies, particularly heart attack or myocardial infarction?
1. Chest pain
2. Pain in the jaw and neck extending to the left arm.
3. Lightheadedness and nausea.
4. Shortness of breath

What variations may occur in the reporting of the symptoms?
1. There may be variations. Not all of the mentioned symptoms may be occurring.
A woman may not say she has chest pain. She may say she is having a heavy sensation or dull ache in her chest.
2. She may say she has pain in the upper abdomen, back, or shoulders.
3. She may experience fatigue, nausea, and/or upper abdomen discomfort.
4. Women are misdiagnosed more than men. Their heart attack symptoms end up being attributed to heartburn or even psychiatric manifestations. Half of women do not have the classic signs of a heart attack.

What are the symptoms of heart rhythm abnormalities?
1. Palpitations
2. Feels like the heart is suddenly beating quickly and irregularly
3. Lightheadedness and feeling faint

What are the symptoms of congestive heart failure?
1. Gradual worsening of the ability to exercise and feeling winded after a short walk
2. Swelling in the legs that requires the use of more pillows to be comfortable sleeping at night

If you could previously walk for 30 minutes every day but now cannot walk a block and become breathless, these are significant findings.
